Pathologic Fractures Secondary to Unicameral Bone Cyst

Abstract
Pathologic fracture is frequently the mode of presentation in a patient with a unicameral bone cysts. We conducted a retrospective review of seventy patients with unicameral bone cysts to determine which cysts were at risk for fracture and if cyst healing was accelerated as a result of the fracture. Seventy patients with 52 pathologic fractures were reviewed. The majority of the lesion arised in the metaphysis of tubular bone. There was a predilection for the proximal end of the humerus, where about 55% of the lesion was situated.
The upper part of the femur was the second most common site. The incidence of patholgic fracture was 92% in the proximal humerus and 52% in the proximal femur (Table 1).
The percentage of bone occupied by the cyst in the transverse plane exceeded 85% on both the anteroposterior and the lateral radiographs in every pathologic fracture patient.
In most cases, although a few months following fracture the roentgenographic appearance was encouraging, the ultimate result was recurrence and even further extension of the original cyst without acceleration of the cyst healing.
